After similar reports more than 10 years ago, we developed strict protocols to prevent any mix-ups in sperm, eggs, or embryos from occurring. Although each patient is assigned a unique identification number that is used to verify gametes before fertilization and to identify embryos before transfer, human error can still cause mishaps. Therefore, we utilize a multi-person review verification process to ensure all samples including sperm, eggs, and embryos are properly verified prior to being labeled and utilized for the creation of embryos.
